               Dear Project Concern/Choice Alum,

We have all been blessed to have experienced a unique educational opportunity through Project Concern or Open Choice, which is now called the Hartford Region Open Choice Program.

CREC, in partnership with the Regional School Choice Office (RSCO), would like to highlight the success of past Project Concern and Open Choice participants/graduates in marketing materials and on the CREC website: www.crec.org/choice.  In addition, CREC & RSCO will sponsor a reunion of Project Concern and Open Choice participants in February 2012.

If you would like to participate in the marketing and/or would like to attend the reunion event in February, please complete the form below by Friday, December 16, 2011.  Forms may be returned via email at rsatchell@crec.org, fax (860) 509-3653 or in the mail at Open Choice, 34 Sequassen Street, Hartford, CT 06106.

Please submit a recent high resolution digital photo with the form.  If you do not have a recent photo, we can arrange to take a photo of you at the February 2012 event.
